Create a Student Referral

Overview

The Add Referral feature allows authorized staff to record student behavior incidents within openSIS. Referrals create a formal record of disciplinary events by capturing the students involved, incident details, behavior classifications, reporting staff, assigned staff, and any administrative decisions. This information helps schools maintain accurate behavior records and ensures incidents are tracked through resolution.


Access the Add Referral Page

  1. Log in to openSIS with appropriate permissions.
  2. From the left navigation menu, select Behavior & Discipline.
  3. Click Add New on the Referrals page.

The referral creation process begins by selecting the student(s) involved in the incident.


Step 1: Select Student(s)

The first screen displays the student list.

You can:

  • Search for students using the search box.
  • Use Advanced Search to locate specific students.
  • Select one or multiple students involved in the incident.
  • Click Next to continue.

Note: A referral can include multiple students when an incident involves more than one participant.


Step 2: Complete the Referral Information

After selecting the students, complete all applicable referral details.

Student Information

Student(s) Involved

Displays the students selected during the previous step. Additional students can also be added if necessary.

Witnesses

Select staff members who witnessed the incident.



Incident Information

Date of Incident

Choose the date on which the incident occurred.

Time of Incident

Enter the time the incident took place.

Possible Motivation

Select the motivation that best describes the reason behind the student's behavior, if known.

Examples may include:

  • Avoid schoolwork
  • Peer pressure
  • Obtain attention
  • Disruption

The available options are determined by your school's configuration.

Location

Select where the incident occurred, such as:

  • Classroom
  • Playground
  • Administration Office
  • Hallway
  • Cafeteria

Available locations depend on your system configuration.


Problem Behaviors

Use this section to classify the student's behavior.

Problem Behavior Major

Select the primary behavior that best describes the incident.

Examples include:

  • Fighting
  • Bullying
  • Leaving class without permission
  • Obscene language or gestures
Problem Behavior Minor

Select any secondary behavior associated with the incident, when applicable.

Both classifications help schools generate accurate behavior reports and identify disciplinary trends.


Details of Incident

Enter a detailed description of the incident using the rich text editor.

You can include:

  • What happened
  • Individuals involved
  • Actions taken during the incident
  • Additional observations

The editor supports standard formatting options to improve readability.


Additional Behavior Information

Depending on your school's configuration, additional behavior fields may be available.

These may include:

  • Custom Behavior
  • Behavior Colour
  • Behaviour Dropdown

These optional fields allow schools to capture institution-specific behavior information.


Assign Staff

The panel on the right side of the page allows you to assign responsibility for the referral.

Assigned To

Select one or more staff members responsible for reviewing or managing the referral.

Reporting Staff

Select the staff member submitting the referral.

This field identifies who reported the incident.


Apply Tags

If referral tags have been configured, they can be applied to categorize the referral.

Tags help identify referrals based on:

  • Priority
  • Follow-up requirements
  • Internal classifications

Tags can also improve filtering and reporting.


Select an Administrative Decision

Choose the appropriate administrative action for the referral, if applicable.

Examples include:

  • Counseling
  • Detention
  • Suspension
  • Seating change
  • Other disciplinary actions configured by the school

Administrative decisions can also be updated later if needed.


Submit the Referral

After reviewing all entered information:

  1. Verify the referral details.
  2. Click Submit.

The referral is saved and becomes available in the Referrals list for future review and follow-up.


Best Practices

  • Record referrals as soon as possible after an incident occurs.
  • Provide clear, factual, and objective descriptions.
  • Select the most appropriate behavior classifications.
  • Assign referrals to the correct staff members for timely action.
  • Apply tags consistently if your school uses them.
  • Update administrative decisions as the referral progresses.
